Schedule your free call with me. We will talk about what led you to seek out changes in lifestyle or diet, and discuss your goals. Then, if you feel like it’s a good fit, the fun begins! (20 minutes)
Personalized Assessment Form
Grab a comfy chair and your favorite beverage (I love an iced latte) and fill out your assessment form. This will help me get a snapshot of all of the details I need to help you get to where you want to be. I look at diet, of course, but I also am interested in other aspects of your lifestyle. My whole-person approach means that I also take things like sleep, stress levels, activity levels, and other factors into account when it comes to creating your customized and flexible plan of care.
We set up our first appointment, where we will discuss the form you filled out between our consultation and our first meeting. We will go over all aspects of your life that are contributing to your overall health— that means an in-depth look at your diet, your medical history, your lifestyle, and the symptoms you might be experiencing. The majority of the session will be time for me to get a clear picture of what your dietary struggles are so together we can create your custom plan. We will end the consultation with setting goals (no more than three— I am all about making sustainable changes) that will support your overarching health outcomes. (60 minutes)
We will determine the frequency of follow-up visits after the initial consultation. Follow-visits are meant to review your progress, address any questions, and address challenges that arose since our last session. Most importantly, these follow-up visits are meant to provide you with the support and accountability to reach your desired goals. (45 minutes)
